Aiming High Home and Community support application

The aim of this service is to enable children and young people with disabilities who need one-to-one support to enjoy play and leisure activities. It is not a childcare or respite care service.

Aiming High Home and Community support funding is either for term time only, or school holiday-only provision (for example, during the summer this could be 2 x 4 hours per week, or term time could be 1 x 4 hour per week) and you may apply for one of these. For holiday funding, applications should be made two months ahead of each main school holiday to allow time to agree dates with families and support staff, if funding is agreed. 

Please be aware that funding is not awarded for the first six months of a change in hours of education provision, eg. starting nursery or school, moving from half days to full days or starting at a residential school. You may apply/reapply after six months, if you would still like support despite the increase in hours of education. Support may come to an end if there is a change in circumstances too e.g. young person starts attending school. 

There is a section on the application form which is to be completed by a person who knows your child/young person in a professional capacity. If you can please nominate a professional such as a Portage worker, Speech or Physiotherapist, teacher or classroom assistant, playgroup leader, health visitor, GP, or anyone else who works with your child, we will then contact them for further details. Please inform the professional that we will be contacting them.
